The three successive governments of Presidents Bush, Obama and Trump have had different approaches to healthcare-related problems in the country, which has seen a number of interventions made to mitigate the problem. Agenda setting and formulation form the first two stages of public policy, which is a set of actions and decisions taken by governments towards the solution of a given problem, in this case, being healthcare. Public policies are based on law hence requires that it goes through a number of stages to allow a government to deal with an issue. The first step is that the problem has to be recognized, then an agenda is set to show the problems the government intends to solve, the policy is then formulated by various decision-makers. The other stage is the adoption of a policy which often happens when congress passes a law or when a president signs an executive order or when the Supreme Court rules on an important case. The policy then moves to the implementation stage, mostly by administrative agencies within the executive and finally into the evaluation stage to determine if a policy has been accomplished and whether it is being carried out efficiently. Various stakeholders are involved in the process of policymaking so that they lobby for the interests of their members. The American Nurses Association, for example, has been at the forefront of advocating for improved healthcare for all as well as the nurses’ welfare (O’Rourke, Crawford & Morris, 2017). In 2014, The Care Coordination Task Force (CCTF) was convened by the leadership of the American Nurses Association and the American Academy of Nursing (AAN), to review major position papers and policy briefs on care coordination. One of the aims was to recommend specific and actionable federal policy priorities to advance the contributions THE HEALTH AGENDA 3 of nurses to effective care coordination (Lamb et al., 2015). When professionals understand the political process, they may facilitate the efforts of their leaders and organizations to promote change in state rules and regulations. From these lobby groups, their voices reach the executive and congress agencies so that they share their perspectives and expertise directly with the policymakers, which provides an arena where these views are incorporated into the government’s agenda. Almost all American Presidents have faced healthcare crises while in the office that have ranged from political scandals, natural disasters, economic calamities and terrorism. In dealing with the problem of affordable healthcare, Presidents Bush, Obama and Trump have made their contributions, which have brought about key changes as far as healthcare’s access and affordability for Americans is concerned. In 2003, President Bush enacted the Medicare Modernization Act, to help Americans access the healthcare they needed at an affordable price as well as infusing transparency and innovation into the healthcare system (Pope, 2017). President Obama would also in 2010, enact the Patient Protection and Affordable Care Act, commonly known as Obamacare, with the aim of lowering the costs of healthcare and to improve the quality of life for those who could not get health insurance from their employers. The reforms would come in to lower the costs of emergency rooms as well as chronic illnesses, which were some of the most expensive healthcare services. When President Trump came into office his policy aimed at putting the American patients first including with a more aggressive approach to deregulation and regulatory reforms in a bid to outsmart any administration before him. His reforms would see the Department of Health and Human Services cut the burden of its regulations by $25.7 billion to become the number 1 agency in terms of regulatory savings (Azar, 2020). Despite their contributions, the question of healthcare continues THE HEALTH AGENDA 4 to be a challenge to many Americans, especially those from the low economic caucus, in terms of coverage and accessibility. One of the areas that should be looked at, without politicization of the whole campaign is to ensure that every American has access to healthcare at an affordable price, and with a proper medical cover. Given a chance to be the President of the United States, I would ensure that all stakeholders within the health sector are involved in an exercise that allows them to give their views concerning how the question of affordable health can be solved once and for all. The reports would include proposals for the way forward, including how the existing policies can be improved to make the experience better for all people. I would then lobby the concerned agencies within the government to ensure that such recommendations are drafted into a policy, which I would task the necessary agencies to implement. This way, everyone from insurance providers, government agencies to the people would give their input towards achieving a policy that is beneficial for all.
