An organization model describes the objectives and the structure of a health care organization in terms of roles, norms, relations between roles, and interactions between roles. It is a comprehensive program that emphasizes a shared focus on excellence, systematically responsive management, evidence-based medicine, measures and goals, improvement of work processes, training, and extensive rewards (IGI Global, 2022). An operational model is both an abstract and visual representation of how a health care organization delivers value to its customers or beneficiaries as well as how an organization runs itself. It dictates where and how the critical work gets done across a health care system. It translates strategy into a structure to move towards a successful future state. Overall, the operating model should address how the organization will fulfill its roles (HealthLeaders, 2018).

Organizational and operational models vary among different health care organizations. For example, in transitional care, its purpose is designed to prevent health complications and re-hospitalizations of chronically ill, elderly hospital patients by providing them with comprehensive discharge planning and home follow-ups. This is done through an organizational model that is coordinated by a master’s-level “Transitional Care Nurse” who is trained in the care of people with chronic conditions. At the time of hospitalization, the Nurse: (i) conducts a comprehensive assessment of the patient’s health status, health behaviors, level of social support, and goals; (ii) develops an individualized plan of care consistent with evidence-based guidelines, in collaboration with the patient and her doctors; and (iii) conducts daily patient visits, focused on optimizing patient health at discharge (Social Programs, 2018). Meanwhile, an operational model was used to determine how the patient’s quality of care can be improved. For example, the nurse will come up with a specific care plan as well as potential risk factors for that patient that focus on reducing negative outcomes and increasing positive outcomes to achieve their health goals (HealthLeaders, 2018).

In home health, care service consists of providing care to patients in their homes. This is done through an organizational model that is coordinated by home health nurses who are trained to treat patients in their homes. The most important aspect of a well-designed home health care agency’s organizational structure lies in its contribution to timely and appropriate care. Clear lines of communication between supervisors and field staff help assure that all information about changes in a client’s condition reaches that personnel charged with overseeing care plans, communicating with clients’ physicians, and screening staff/client interactions for potential liabilities (Mott, 2018). Additionally, an operational model is used in-home care to ensure the needs of staffing, operations, and the patient/patient’s family’s needs are being met. For example, an operational model can be used to provide staffing strategies for scheduling the at-home nurses. When there is a high volume of patients needing at-home care, the operational model can be utilized to hire temporary home health nurses.

In long-term care, they provide rehabilitative, restorative, and/or ongoing skilled nursing care to patients or residents in need of assistance with activities of daily living. Government-run long-term care facilities are transparent in their organizational model structures. They operate through state agencies that provide an annual budget and are run by an appointed administrator who must make decisions based on the annual funds provided (Ray, 2018). However, the operational model is designed to meet a variety of services to better a person’s health or personal care needs during a short or long period of time. These services help people live as independently and safely as possible when they can no longer perform everyday activities on their own (U.S. Department of Health and Human Services, 2018).
