Background:
Congestive heart failure (CHF) is a serious heart condition characterized by failure of the heart muscles to efficiently pump blood as it should to all parts of the body. It remains one of the leading causes of mortality and morbidity globally, with the 5-year mortality rate at approximately 45% for women and 60% for men (Surikova et al., 2020). According to Breathett et al. (2019), heart failure is also one of the leading causes of hospital readmission in the U.S. Heart failure education is perceived as a means of empowering patients to improve outcomes, as patients need to understand and appreciate the relevant treatment requirements. The importance of follow-up after hospital discharge is recognized in National Institute for Health and Care Excellence guidance (Vernon et al., 2019), to reduce the rate of readmission, and given that heart failure is among the leading causes of hospital readmission, targeted interventions can be an effective way of reducing the readmission rate in CHF patients.
PICOT Question:
For patients with a recent hospitalization for CHF exacerbation (P), does receiving nursing care and education through a home health agency (I) compared to not receiving nursing care and education (C) reduce re-hospitalizations (O) after 3 months (T)?
