Case Study: End of Life Decisions

George is a successful attorney in his mid-fifties. He is also a legal scholar, holding a teaching post at the local university law school in Oregon. George is also actively involved in his teenage son’s basketball league, coaching regularly for their team. Recently, George has experienced muscle weakness and unresponsive muscle coordination. He was forced to seek medical attention after he fell and injured his hip. After an examination at the local hospital following his fall, the attending physician suspected that George may be showing early symptoms for amyotrophic lateral sclerosis (ALS), a degenerative disease affecting the nerve cells in the brain and spinal cord. The week following the initial examination, further testing revealed a positive diagnosis of ALS. Case Study on Death and Dying

ALS is progressive and gradually causes motor neuron deterioration and muscle atrophy to the point of complete muscle control loss. There is currently no cure for ALS, and the median life expectancy is between 3 and 4 years, though it is not uncommon for some to live 10 or more years. The progressive muscle atrophy and deterioration of motor neurons lead to the loss of the ability to speak, move, eat, and breathe. However, sight, touch, hearing, taste, and smell are not affected. Patients will be wheelchair-bound and eventually need permanent ventilator support to assist with breathing.

George and his family are devastated by the diagnosis. George knows that treatment options only attempt to slow down the degeneration, but the symptoms will eventually come. He will eventually be wheelchair-bound and be unable to move, eat, speak, or even breathe on his own.

In contemplating his future life with ALS, George begins to dread the prospect of losing his mobility and even speech. He imagines his life in complete dependence upon others for basic everyday functions and perceives the possibility of eventually degenerating to the point at which he is a prisoner in his own body. Would he be willing to undergo such torture, such loss of his own dignity and power? George thus begins inquiring about the possibility of voluntary euthanasia.

In the above case given, it can be noted that George is suffering with ALS and has different religious viewpoints, which create various reasoning for George’s current situation. In the light of the Christian narrative, George believes that God is present, and that everything which has happened to George is a consequence to his own actions. Moreover, it is important to understand that different churches have different schools of thought and belief systems, which they teach differently about the wrath and pain that are experienced by their constituents in general. George’s current pain can be explained by the Christian’s world view as somewhat God’s intention to let him know the pain the same as Jesus experienced in his visit to the earth as a man. Nevertheless, George’s experience of the pain is expected to make him much closer to God his faith is strengthened in prayer. On the other hand, for some Christians, George’s suffering is altogether a different belief in which they would believe that the George is experiencing the pain due to the sins committed by him in the previous life. In either case there is positive outlook shown through Christian world view, if the pain is the God’s will, then it will teach George to be steadfast and appreciate the faith of the God. As a Christian, George believes that all of us are created differently with different thinking ability and as human beings, we are required to take care of our lives and should never take this life for granted. Based on my world view, even if I am a Christian, if I were in George’s situation and facing the imminent suffering and disability associated with ALS, I would consider voluntary euthanasia. Even though voluntary euthanasia is against Christianity, it is difficult to watch individuals suffer as a nurse and personally I would not want to experience such pain and suffering if there is an option of euthanasia. Although it is not morally right to choose euthanasia, the Bible states that God is forgiving and there is no single sin that cannot be forgiven.
