In the United States, 15 million central venous catheter (CVC) days occur in intensive care units each year (Mermel, 2000). Studies have variously addressed catheter-related bloodstream infections (CRBSI) and have found them to increase hospital costs and length of stay. What is the current data on that now? Google these data and recommendations for practice. Choose organization (.org) websites you will need to include in your work. Check out the academic library for sources within the last 5 years. To improve patient outcome and to reduce healthcare costs, there is considerable interest by healthcare providers, insurers, regulators and patient advocates in reducing the incidence of these infections. This effort should be multidisciplinary, involving healthcare professionals who order the insertion and removal of CVCs, those who insert and maintain the catheters, infection control personnel, healthcare managers including CEOs and those who allocate resources, and patients who are capable of assisting in the care of their catheters. The goal of an effective prevention program should be the elimination of or the reduction to as low a rate as possible given the specific patient population, CRBSIs from all patient care areas. 2 With this information in mind, conduct a thorough review of the literature and answer the following questions: 1. What are the current clinical guidelines for caring for CVCs? 2. What care approach is used in your (or a colleague’s) clinical setting? Social factors such as race, ethnicity, and socioeconomic status are strongly associated with quality of health and health risks. Some specific factors include education, nutrition, housing, transportation, clean water and air, and health insurance (HealthyPeople.gov, 2018). Research by the Centers for Disease Control (2013) finds persistent levels of disparity between some population groups. For example, differences were found between racial and ethnic groups for all categories measured. Socioeconomic status was found to be a risk factor for diseases such as diabetes and hypertension, as well as predictor for a lack of preventive medicine (e.g. colonoscopies, mammograms, and vaccinations). The National Institutes for Health (2013) expect minority populations to comprise about half of the US population in the next thirty years, which will place more strain on resources in underserved communities, further restricting access. The Robert Wood Johnson Foundation offers suggestions to health care workers for supporting a goal of decreasing health outcome disparities:
• Adopt a new set of vital signs that include non-medical factors (e.g. living conditions, access to food) • Commit to providing extra service to underserved populations • Prevent bias in diagnosis and treatment • Prevent bias in research (age, gender, ethnicity representation) Central venous catheter
