Inmates get transferred from facility to facility, and at times, the records are missing and have to ship at a later date and not by mail. We have to wait on New Gains- intake day (When A new group of inmates arrives VIA bus, which is usually once a week on Thursday. The elimination of duplicate charting, which can lead to errors in documentation. There are different assessment forms used within the institution to complete when inmates get ill or injured. And if an inmate has multiple issues, then various assessment forms have to be completed.
