What is EMR: Electronic Medical Record Pros- Always having access to the medical chart. Inmates get transferred from facility to facility, and at times, the records are missing and have to ship at a later date and not by mail. We have to wait on New Gains- intake day (When A new group of inmates arrives VIA bus, which is usually once a week on Thursday. The elimination of duplicate charting, which can lead to errors in documentation. There are different assessment forms used within the institution to complete when inmates get ill or injured. And if an inmate has multiple issues, then various assessment forms have to be completed. The use of the EMR would condense the forms. Types of Assessments: • • • • • • • • • • • Ear, Nose & Throat Wound Post use of force Musculoskeletal Confinement Skin Respiratory Chest Pain Head Injury Altered mental status Neurological Prevention of Medication Errors- The medication administration Record (MAR) is handwritten when medications are renewed; dosages changed, discontinued, held, or tapered. MARs often get miss placed, handwriting is illegible, duplicate MARs lead to dosing errors and missed does because when inmates change housing location, the MAR is not moved for days. Because the nurse didn’t, take the time to research the housing location. This leads to untreated HTN-Hypertension, Diabetes, and seizure disorders, which could lead to death, illness, or injury.
