The design of adult acute care units in U.S. hospitals. Journal of Nursing Scholarship, 41(1), 79–86. Overview: This descriptive study examined the current state of hospital unit design characteristics recommended by the Agency for Healthcare Research and Quality (AHRQ) in 81 adult medicalsurgical units and 56 intensive care units in six metropolitan areas. The AHRQ recommends that the following unit design characteristics positively impact patient outcomes: single rooms, work areas for staff that are not a long distance from the bedside, frequent staff hand hygiene stations, certain types of unit configuration, percentage of private rooms, and presence or absence of carpeting. The purpose of this study is to provide a benchmark and to assess nursing environments. Data were collected by observation, measurement, and interviews. The researchers conclude that few of the hospital units met the AHRQ recommendations. Further research is required to expand understanding of these design elements, their interaction, and impact on outcomes. Application: Health care organizations are much more than a description of the organization. They are also physical buildings. Transforming the Work Environment of Nurses (2004) pertain to design of work and workspace to prevent and mitigate errors. This study on unit design elements relates to the IOM work, which is referenced in the study. There are many factors and elements that impact the quality of care and design is one of them. Historically nurses typically have had limited input into design of units, but more hospitals are including nursing management and staff nurses in the decision making process when facilities are renovated or new buildings are built. Intertwined within these critical elements is the recognition of the importance of autonomy, responsibility, delegation, and accountability. “Autonomy in clinical decision making occurs whenever a nurse makes an independent judgment about the presence of a clinical issue and then provides the resolution to nursing care” (RitterTeitel, 2002, p. 32). Autonomy requires competence and skills that focus on the nurse–patient relationship. It also means that there needs to be an organized assessment method to determine patient care needs and reassigning staff. Nurses also have the right to consult with others as professionals when they provide or manage care. Autonomy, control, and decision making are related. “Professional practice implies control over the terms of the work but also control over its content and regulation of its standards” (RitterTeitel, 2002, p. 33). Nurses who feel that they have autonomy know that they have the right to make decisions in their daily practice and also actively participate in developing organizational policy and change. Staff autonomy, however, does not work in organizations in which leaders are authoritarian and when centralized decision making and control are key characteristics of the organization. This situation will quickly lead to conflict. In addition, the work environment must be conducive to collaboration with physicians and all relevant staff, as is discussed in Chapter 12 . “Responsibility refers to being entrusted with a particular function” (RitterTeitel, 2002, p. 34). A nursing practice model that does not address responsibility will not be effective. Along with this is the need to clearly recognize the importance of delegation. “Delegation involves transferring responsibility for the performance of the task from one person to another” (RitterTeitel, 2002, p. 34).
Delegation is discussed in more detail in Chapter 14 . Accountability is a term that is typically found in job descriptions and descriptions of organizational structure. In nursing it is particularly important to recognize that “accountability is the acceptance of responsibility for the outcomes of care” (RitterTeitel, 2002, p. 34). Nurses need to know that when they provide patient care their work has relevance—it must reach outcomes. Magnet hospitals are discussed in Chapter 6 as examples of organizations that exemplify these characteristics. The AONE elements and these characteristics, such as accountability, need to be considered when nursing practice models are assessed. Models of care are developed to support or enhance professional practice, and by considering these elements and characteristics the models will be more effective.
