Additionally, as the advanced practice nurse, it is your responsibility to ensure that when prescribing prescription drugs, you adhere to the ethical and legal principles set forth for prescribing drugs as an added layer of protection and safety for the patients you will treat.
This week, you will analyze factors that may influence pharmacokinetic and pharmacodynamics processes of a patient and assess the details of a personalized plan of care that you develop based on influencing factors and patient history. You will also evaluate and analyze ethical and legal implications and practices related to prescribing drugs, including disclosure and nondisclosure, and analyze the process of writing prescriptions to avoid medication errors.

Discussion: Pharmacokinetics and Pharmacodynamics
As an advanced practice nurse assisting physicians in the diagnosis and treatment of disorders, it is important to not only understand the impact of disorders on the body, but also the impact of drug treatments on the body. The relationships between drugs and the body can be described by pharmacokinetics and pharmacodynamics.
Pharmacokinetics describes what the body does to the drug through absorption, distribution, metabolism, and excretion, whereas pharmacodynamics describes what the drug does to the body.
When selecting drugs and determining dosages for patients, it is essential to consider individual patient factors that might impact the patient’s pharmacokinetic and pharmacodynamic processes. These patient factors include genetics, gender, ethnicity, age, behavior (i.e., diet, nutrition, smoking, alcohol, illicit drug abuse), and/or pathophysiological changes due to disease.

My current work station is in the post-surgical unit at the Veterans Affairs (VA) Healthcare System. I encountered a post-emergency right hip arthroplasty due to mechanical fall at home. The patient is 75 years old, male, BMI 24.8 with significant “shahadah” or declaration of Islam faith. The veterans served the army post -Vietnam war. The medical history includes hypertension, vitamin D deficiency, and adjustment disorder. The patient is alert times three, cooperative, tends to hold off on pain medicine, and very involved with his care. The orthopedic orders include the use of an incentive spirometer, venodyme, turning, intravenous Acetaminophen 1gm every six hours, intravenous Ketorolac for breakthrough pain, Heparin 5,000 units subcutaneous every 8 hours and continue all home medications in the morning. The factors that will influence pharmacokinetic for the veterans is metabolism due to old age. The enzymatic changes of drug structure known as biotransformation or drug metabolism. The six common outturns for metabolism include faster renal drug excretion, drug inactivation, therapeutic action increase, prodrugs activation, higher toxicity, or decrease toxicity. The decrease in drug metabolism for an older patient is significant; hence medication dosages must be lowered to prevent drug toxicity (Rosenthal, & Burchum, 2018). The pharmacodynamics process that will affect the veterans is the drug-response relationship, specifically relative potency. The biochemical and physiologic effects of medications on the body and the molecular mechanisms wherein effects are created by pharmacodynamics. The use of Heparin subcutaneous post-operative is the standard of care in most orthopedic cases except with bleeding or clotting disorder. The pharmacokinetics of Heparin’s metabolism may be lower due to the patient’s advancing age with a higher chance of bleeding. While the pharmacodynamic of the relative potency of Heparin for the older patient may need to be of minimum dosage due to BMI. The situation stated above seems relative benign until Heparin subcutaneous was discussed. The patient was very involved with his care asked his daughter to searched for his hospital medications.
