Running head: DIABETES SELF-CARE PLAN Diabetes Self-Care Plan Yekaterina Kremleva NSG/482 University of Phoenix Professor Karen Harriman 1 DIABETES SELF-CARE PLAN 2 Diabetes Self-Care Plan Community health response to JK’s situation will require the preparation of a meal plan, analysis of the means to access food, and support services for the patient. Starting with the meal plan, the patient has recently been diagnosed with diabetes and may lack relevant nutrition information (Alhaik et al., 2019). Therefore, patient education will be conducted. JK’s community can be compared to the community in Pima County. In comparison, 18% of people in Pima County, live in food deserts (Mittan, 2021). Moreover, the rates of diabetes are also very high in Pima County with 12% of the population having diabetes (Healthy Pima, 2015). In order to access healthy food options in food deserts in Pima County, JK would need a minimum of $10 daily hence a weekly minimum of $70. In Pima County, the patient can access fresh food markets such as Sprouts and Heirloom farmers’ markets. The patient will need several support services for effective diabetes, asthma, and hypertension management. Promoting Healthy Communities
The patient is eligible to Health Homes Medicaid program which helps people with more than one chronic condition to access coordinated care (Health Homes, n.d.). The patient requires support for assistance in meal preparation and health maintenance. JK’s comorbidities reduce her self-care efficacy hence the need for living assistance for her to effectively manage the conditions. JK also needs mental health support because due to multiple chronic conditions, she is at risk of mental disorders (Hajat & Stein, 2018). The patient can access these services through Medicare Part B. DIABETES SELF-CARE PLAN 3 References Alhaik, S., Anshasi, H. A., Alkhawaldeh, J. F., Soh, K. L., & Naji, A. M. (2019).
