Leininger’s Transcultural Nursing Theory

Transcultural nursing theory is one of the models suggested for application in the practice of healthcare facilities. The theory was developed by Leininger, who notes that patients visiting hospitals have diverse cultural peculiarities. Hence, there is the need for cultural competence, awareness, and sensitivity practiced by nurses and other clinical service providers. The theorist postulates that the patients’ culture influences their health practices and care decisions; hence, nurses and physicians need to establish these influences before designing a care plan (McFarland & Wehbe-Alamah, 2019). The central phenomenon raised in the theory is that care should be culture-centered, with patients being allowed to decide on the treatment plans aligned with their beliefs, values, and norms. According to this theory, patients must be placed at the center of the healthcare facilities’ care process.

Nurses need to be sensitive to their clients’ culture and ensure that they do not execute any action likely to violate patients’ autonomy. The theory is also guided by beneficence and autonomy nursing principles, which require nurses to provide beneficial services and allow their clients the opportunity to select appropriate treatment approaches (Sagar & Sagar, 2018). Therefore, nurses must first establish the belief system and other culture-related preferences of their clients and incorporate these elements in the clinical plan. Additionally, the transcultural nursing theory is grounded on the idea that different cultures adhere to diverse care patterns. Therefore, each patient selects treatment decisions and care plans they are familiar with while perceiving any other care plan as ineffective and harmful. Therefore, the healthcare personnel should perform cultural evaluation of the clients, taking notes regarding the beliefs and norms surrounding patients’ healthcare practices (Sagar & Sagar, 2018). These findings should be integrated into the care plan to ensure that medical procedures are acceptable and easy to follow upon discharge from healthcare facilities. The study would also provide cultural knowledge to care providers, thereby improving their service delivery competence.

The theorist makes several assumptions during the model’s development. First, she contends that different cultures have diverse knowledge and perceptions about healthcare practices. According to the theory, the care practice starts from families and communities, and individuals are highly likely to adopt healthcare practices they became familiar with during their childhood (Smith & Parker, 2015). Differentiated cultural approaches to care call for nurses to ensure that they use an individualized approach that pays attention to the clients’ culture while ensuring that the primary nursing principles are upheld.

Furthermore, Leininger assumes that some factors influence patients’ healthcare choices; hence, there is the need for nurses to identify and integrate them into the care practice. For instance, the theorist argues that education, kinship, and religion are factors that dictate individuals’ health choices (McFarland & Wehbe-Alamah, 2019). For instance, some religions require their followers to refrain from blood transfusion, consume certain foods, or even avoid healthcare facilities when sick. Therefore, nurses need to consider these factors, addressing inapplicable ones and integrating favorable ones into the core processes of care.

Finally, the theorist assumes that nurses are only beneficial to patients when they understand their preferences, beliefs, expressions, and values. The main idea presented by Leininger is that clients’ culture influences clinical outcomes. The same argument is presented by Im and Lee (2018) stating that nurses with prior knowledge about the patients’ culture and preferences are likely to provide the most effective care. Therefore, it is difficult for nurses, based on the theorist’s arguments, to realize positive clinical outcomes when the clients’ beliefs and values are not integrated into the care process.
